PREPARE for YOUR Care (PREPARE) is an evidence-based online resource that helps people begin the important process of planning for medical decision-making. Using video stories, PREPARE helps people explore their personal wishes and learn how to discuss them with family, friends, and medical providers. Its website offers multilingual, easy-to-read advance directives for US states and Washington, D.C., as well as a program to support caregivers. In addition, it provides simple conversation guides, pamphlets, and Toolkits to initiate advance care planning discussions and to conduct group medical visits or community events.
PREPARE advance directives are available in English and Spanish for U.S. states, Traditional Chinese for 13 states, and additional languages for California and Minnesota. As of January 2026, the California advance directive is offered in Arabic ( العربية), Armenian (հայերեն), Traditional Chinese (中文), English, Hmong, Khmer (ខ្មែរ), Korean (한국어), Persian (فارسی), Russian (русский), Somali (Soomaali), Spanish (español), Tagalog, and Vietnamese (Tiếng Việt).
